Common Interview Questions for Drug Safety Executive (Case Processing and Aggregate Report)

Prepare for your interview by reviewing these frequently asked questions. Click on a question to reveal the answer.

What are the key steps involved in processing an individual case safety report (ICSR) from receipt to database entry?

The key steps include receipt and acknowledgment of the case, verification of source documents, assessment of seriousness and expectedness, data extraction and entry into the safety database, generation of a case number, performing data quality checks, raising queries for missing information, and finally, case closure after all required actions are completed.

How do you ensure data integrity when entering adverse event information into a safety database?

Data integrity is maintained by double‑checking source documents, using standardized terminology (MedDRA, WHO‑DRUG), following SOPs for data entry, performing automated and manual validation checks, documenting any discrepancies, and conducting periodic audits to verify accuracy.

Explain the differences between PADER, PSUR, and PBRER reports.

PADER (Periodic Adverse Drug Experience Report) is a US‑focused periodic safety report summarizing adverse events. PSUR (Periodic Safety Update Report) is an EU‑required report providing a comprehensive safety evaluation over a defined period. PBRER (Periodic Benefit‑Risk Evaluation Report) is an ICH‑aligned report that assesses both benefits and risks of a medicinal product, incorporating a benefit‑risk analysis.

What regulatory guidelines govern the preparation of aggregate safety reports in India?

In India, aggregate safety reports are governed by the CDSCO guidelines, which align with ICH E2C(R2) for Periodic Safety Update Reports (PSUR) and the Indian Pharmacopoeia for adverse event reporting. Companies must also adhere to the Schedule Y requirements for clinical safety reporting.

Describe how you would handle a serious adverse event (SAE) that lacks complete source documentation.

I would first raise a data clarification query to the reporting source (investigator, site, or vendor) requesting the missing information. Simultaneously, I would document the query in the safety database, flag the case as pending, and ensure that the SAE is reported to the regulatory authority within the required timelines using the best available information.

What is a compliance tracker and how is it used in pharmacovigilance?

A compliance tracker is a tool (often a spreadsheet or database module) that monitors the status of all required safety submissions, including aggregate reports, to ensure they are completed on schedule. It records submission dates, regulatory authority feedback, corrective actions, and helps identify any deviations for timely remediation.

How do you perform a root‑cause analysis for data discrepancies in safety reports?

Root‑cause analysis involves identifying the point of error by reviewing the source documents, data entry logs, and SOPs. Techniques such as the 5 Whys or Fishbone diagram are applied to trace the origin of the discrepancy, followed by implementing corrective actions and updating SOPs to prevent recurrence.

What strategies would you use to stay updated with evolving global pharmacovigilance regulations?

I regularly attend industry webinars, subscribe to regulatory newsletters (FDA, EMA, CDSCO), participate in professional networks, review ICH guideline updates, and complete continuing education courses on pharmacovigilance.

Explain the importance of signal detection in drug safety and your role in it.

Signal detection identifies potential safety concerns early by analyzing aggregated adverse event data. My role includes ensuring high‑quality case data, contributing to data mining activities, reviewing safety trends, and communicating any emerging signals to the safety assessment team for further evaluation.

How would you manage multiple aggregate report deadlines across different regions?

I would maintain a master calendar with all regional deadlines, prioritize tasks based on submission dates, allocate resources accordingly, and use the compliance tracker to monitor progress. Regular status meetings with cross‑functional teams ensure alignment and timely completion.

What is MedDRA and why is it critical in pharmacovigilance?

MedDRA (Medical Dictionary for Regulatory Activities) is a standardized medical terminology used globally to code adverse events. It ensures consistent data capture, facilitates regulatory submissions, and enables effective signal detection across different regions.

Describe your experience with safety database systems such as Argus or Arisg.

I have 3‑4 years of hands‑on experience with Argus, performing case entry, query management, and report generation. I am proficient in navigating the system, customizing fields, and generating audit trails for regulatory inspections.

How do you ensure confidentiality and compliance with data privacy regulations when handling patient information?

I follow GDPR and local privacy laws by anonymizing patient identifiers, using secure encrypted systems, restricting access based on role, and adhering to company SOPs for data handling and storage.

What steps would you take if a regulatory authority raises a query on an aggregate report you submitted?

I would review the query, retrieve the relevant data and documentation, prepare a detailed response addressing each point, involve the appropriate functional teams for clarification, and submit the response within the stipulated timeframe.

Explain the concept of CAPA and its relevance in pharmacovigilance.

CAPA (Corrective and Preventive Action) is a systematic approach to identify, correct, and prevent recurring issues. In pharmacovigilance, CAPA ensures that data quality issues, process gaps, or compliance deviations are resolved and prevented, maintaining the integrity of safety data.

How would you handle a situation where a vendor consistently provides incomplete safety data?

I would document the recurring issues, discuss them with the vendor to understand root causes, provide clear data requirements, and if necessary, involve the procurement team to enforce contractual obligations or consider alternative vendors.

What are the key components of a risk management plan (RMP) related to drug safety?

Key components include safety specifications, pharmacovigilance plan, risk minimization measures, and a detailed evaluation of the benefit‑risk balance, along with post‑marketing safety studies and monitoring strategies.

Describe how you would prepare a safety summary for a new product dossier.

I would collate all safety data from pre‑clinical, clinical, and post‑marketing sources, perform a comprehensive analysis of adverse events, assess causality, summarize benefit‑risk conclusions, and format the summary according to regulatory guidelines for inclusion in the dossier.
